2009-2010 Medical Release, Permission and Registration Form

Registration for the 2009-10 club year is
Sunday, July 26th OR Sunday, August 16th 2009

IMPORTANT REGISTRATION INFORMATION

Returning families PLEASE NOTE: Prefilled registration forms will be available for your review when you come to register. Please update these forms only. You do not need to fill out new forms.
New families PLEASE NOTE: Please download and fill out the registration form and release forms prior to registration day and bring them with you to registration day.
If you are unable to come to registration day, please arrange for a responsible adult to bring your completed forms along with payment to registration day.

Please register early. We will not take registrations the first night of club.
Do not drop off or mail registrations to the church. Please register in person.

Registration times:

2:00 - 3:00 pm Leader's children registration
3:00 - 4:30 pm Returning clubber registration
4:30 - 5:30 pm New clubber registration

Registration cost is $40 per child which includes one book, all awards and most activities.

Cubbie or Sparks vest - $15
T&T, Trek or Journey Shirt - $20

Space is limited to leader availability as the Lord provides. We maintain a 5:1 child to leader ratio.

Clubs fill up quickly. Please come early to ensure that your child(ren) get enrolled. Once a club is filled, we will start a waiting list.

The cost for enrolling is $40 per child. Vests for Cubbies and Sparks are $15. Shirts for Truth and Training, Trek and Journey cost $20. This cost includes a uniform shirt / vest and the first handbook. Additional handbooks cost $10. Please specifiy each child's shirt size and handbook Bible version choice (NIV or NKJV).

We will buy back gently used vest and shirts without awards attached for $5.00 each and sell them back to families in need.
